When you apply for permanent residence, one of the pieces of evidence that you must submit is the I-693, "Medical Examination of Alien Seeking Adjustment of Status." I am often asked if an exam by the immigrant's own doctor is acceptable for this green card requirement; actually, the exam has to be performed by a doctor on USCIS's list of Civil Surgeons.
